Finance — Lead: Lab budgets need a test-level check, not a blanket 15% cut

For medical groups with laboratory revenue paid under Medicare's Clinical Laboratory Fee Schedule, the 2027 planning question is which tests drive revenue. CMS released preliminary rates on September 21 using updated private-payer information. The annual reduction limit for an individual clinical diagnostic laboratory test is 15% through 2029; that ceiling does not mean every test will lose 15%.

Today's Signal

Finance — Lead: Lab budgets need a test-level check, not a blanket 15% cut

For medical groups with laboratory revenue paid under Medicare's Clinical Laboratory Fee Schedule, the 2027 planning question is which tests drive revenue. CMS released preliminary rates on September 21 using updated private-payer information. The annual reduction limit for an individual clinical diagnostic laboratory test is 15% through 2029; that ceiling does not mean every test will lose 15%.

CMS expects to finalize rates in November, with payment changes beginning January 1, 2027. Comments on the preliminary rates and supporting data are due October 21. This is a planning window, not a final fee schedule. CMS announcement — September 21, 2026; CMS deadline notice — September 24, 2026.

The business implication: a group's test mix matters more than the headline percentage. A useful finance conversation is whether its largest Medicare laboratory revenue lines have been compared with the preliminary prices, with unresolved assumptions kept separate. The evidence does not establish a uniform reduction in total practice revenue or a change to every physician service.

Revenue Cycle Management — Texas: submitter-account access needs an owner

TMHP plans to deactivate electronic data interchange submitters on December 1, 2026, when their submitter accounts have not been accessed since November 30, 2025. For practices using TMHP submission workflows, the immediate question is who owns the relevant account and can confirm its access history—including when an outside billing partner handles submissions. TMHP notice — updated September 23, 2026.

The notice concerns submitter access; it does not announce a provider-enrollment change. It also does not explain what activity counts as account access or how reactivation will work. Those are questions for TMHP before assuming an automated submission or an occasional backup workflow keeps an account active.

Operations — Cyber oversight reaches beyond phishing training

A phishing incident can expose weaknesses in account ownership and offboarding, not only employee awareness. In its September 17 settlement announcement involving California-based Ambry Genetics, HHS cited potential shortcomings in risk analysis, terminating access, and unique user identification. Ambry paid $700,000 and agreed to a corrective action plan monitored for two years. HHS settlement announcement — September 17, 2026.

For multi-provider practices, the operating question is whether someone can demonstrate who owns access removal and individual user identification across systems containing electronic protected health information. This is a lesson from a specific federal enforcement settlement—not a new HIPAA rule, a Texas-only requirement, or a finding about other practices.

What to Watch

  • The final 2027 laboratory prices, expected in November, and whether a group's high-volume tests differ materially from preliminary assumptions.
  • TMHP clarification of qualifying account activity and the process for restoring deactivated submitter access.
  • The rural Texas funding recipients' specific projects and participation arrangements before inferring an opportunity for an individual medical group.
