Today's Signal

Finance

No worthwhile Signal in this section. The Texas Register index (current issue dated October 16, 2026) listed no health items, though issue contents were not opened; the HHS-OIG newsroom showed nothing newer than June 2026; the DOJ news page returned no readable content; and a Becker's deal search turned up nothing dated this month that advances past recent editions. The main finance-adjacent item is a federal ownership bill with "negligible" 2026 prospects, listed below.

RCM — Lead: Check Medicaid and QMB status before billing Medicare cost sharing

The MLN Connects newsletter of October 22, 2026 tells providers that a dually eligible patient who loses Medicaid, including Qualified Medicare Beneficiary (QMB) status, may no longer have QMB cost-sharing protections. CMS says to verify Medicaid eligibility, QMB status and other coverage before billing or charging cost sharing. Original Medicare providers can check through HETS; Medicare Advantage plans will tell providers about any billing protections that apply.

The same issue says that starting October 1, 2026, federal Medicaid funding generally covers only certain noncitizens: lawful permanent residents, Cuban/Haitian entrants and COFA migrants, with some exceptions. It says section 71109 is the October 1 Medicaid funding provision, that section 71201 of the Working Families Tax Cut law limits Medicare entitlement and enrollment to the same groups, and that Medicare terminations under the new rules occur after section 71109 is implemented. The newsletter gives no separate date or count for the Medicare terminations.

Why it matters: a patient's coverage can change between visits. A practice that bills coinsurance on a stale QMB assumption, or holds balances on one that has lapsed, is working from the wrong eligibility record. The newsletter is guidance on checking, not a new payment rule.

Who and when: Medicare-billing practices with dually eligible patients, nationally. The Medicaid funding change applies from October 1, 2026; Medicare terminations follow its implementation, and no date is stated. The newsletter does not break this out for Texas.

Operations

No worthwhile Signal in this section. CMS, TMHP and BCBSTX notices checked today were mostly facility-focused, system downtime or training items without a clear operating effect on multi-provider medical practices.

Worth Your Time

  • RCM — Most medical groups adding RCM automation amid a wave of payer-driven demands — MGMA Stat, October 21, 2026. In an MGMA poll of 214 applicable responses, 63% of medical group leaders said their revenue cycle operations are moving toward more automation in 2026; 11% said toward more manual work. This is a trade-association survey, not a payer or regulator source.
  • RCM — Reimbursement Policy Updates — Blue Cross and Blue Shield of Texas, October 20, 2026. BCBSTX lists updates to four reimbursement policies: Observation Services (effective Dec. 31, 2026), Medical Record Documentation (Oct. 2), Physical Medicine and Rehabilitation (Dec. 31) and Intraoperative Neurophysiology Monitoring (Oct. 9). The notice does not say which products apply or what changed, so check the policy documents.
  • Finance — How proposed restrictions on corporate practice of medicine could affect hospitals — HFMA, October 16, 2026. HFMA describes the federal Stop Corporate Takeovers of Physicians Act of 2026, which would limit practice ownership by insurers and private equity, restrict MSO control and ban most physician noncompetes. HFMA says immediate-term prospects are "negligible." It is a proposal, not law.

What to Watch

  • Clinical Laboratory Fee Schedule comments on CY 2027 preliminary determinations for codes with no applicable information are due November 19, per the MLN Connects newsletter.
  • Whether CMS gives a date for the Medicare terminations tied to the new noncitizen rules.
  • The effective dates of the BCBSTX reimbursement policy updates, December 31, 2026 for two of them.
